Reversible acute axonal polyneuropathy associated with Wernicke-Korsakoff syndrome: impaired physiological nerve conduction due to thiamine deficiency?
نویسندگان
چکیده
Acute axonal polyneuropathy and Wernicke-Korsakoff encephalopathy developed simultaneously in three patients. Nerve conduction studies (NCS) detected markedly decreased compound muscle action potentials (CMAPs) and sensory nerve action potentials (SNAPs) with minimal conduction slowing; sympathetic skin responses (SSRs) were also notably decreased. Sural nerve biopsies showed only mild axonal degeneration with scattered myelin ovoid formation. The symptoms of neuropathy lessened within two weeks after an intravenous thiamine infusion. CMAPs, SNAPs, and SSRs also increased considerably. We suggest that this is a new type of peripheral nerve impairment: physiological conduction failure with minimal conduction delay due to thiamine deficiency.

A case report: Non-alcoholic Wernicke encephalopathy associated with polyneuropathy
We report a rare case of non-alcoholic Wernicke encephalopathy (WE) with polyneuropathy. A 24-year-old woman who had recently served a 4-month prison sentence and underwent a short period of dieting manifested slow response, weakness, language disorder and amnesia. Brain magnetic resonance imaging (MRI) revealed typical lesions of WE. Examination of nerve conduction velocity revealed sensory-mo...

Sir, We read with interest the recent paper of Gibb et al. (1985) in which reversible coma in Wernicke's encephalopathy was reported. Treatment with thiamine was successful in that the patient regained consciousness but unsuccessful in preventing the residual amnestic defect of Korsakoff's encephalopathy. In a similar report (Wallis et al., 1978) four comatose patients with the Wernicke syndrom...
متن کاملThiamin deficiency as a cause of reversible cor pulmonale.
Thiamine deficiency may present four classic clinical forms: peripheral polyneuropathy, anorexia and muscular weakness (dry beriberi); high output heart failure with signs of congestion (wet beriberi); beriberi associated with shock (Shoshin beriberi) and Wernicke's encephalopathy. In this report we describe a picture that is suggestive of severe pulmonary hypertension and cor pulmonale, with j...
متن کاملWernicke-Korsakoff syndrome
Wernicke-Korsakoff syndrome is a manifestation of thiamine (vitamin B1) deficiency, usually secondary to alcohol abuse, although there other non-alcohol related causes, such as bariatric surgery, and extreme dietary restriction. The syndrome is a combined manifestation of the short-lived but acute Wernicke’s encephalopathy (WE), followed by chronic Korsakoff syndrome. It is generally accepted t...
متن کاملWernicke–Korsakoff syndrome in a patient with self-neglect associated with severe depression
Wernicke-Korsakoff syndrome is a neuropsychiatric disorder resulting from thiamine deficiency and commonly associated with chronic alcoholism, but we describe the first case report resulting from self-neglect associated with depression.
